Your First Visit


Please assist us at the time of your initial visit to the office by providing the following information:

  • Your referral slip and x-rays from your family dentist.
  • A list of medications you are currently taking (Click here to fill out your Patient Registration and Medical History Form online).
  • If you have dental insurance, please bring any forms or insurance cards with you to the appointment.

Please Note: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation appointment.

Please notify the office if you have a medical condition or concern prior to your initial appointment (e.g. artificial heart valves or joints, heart murmurs requiring premedication, severe diabetes, or hypertension.)

If your family dentist has taken x-rays, you may request that they be forwarded to us.

At the initial visit, we will thoroughly review your medical and dental history, and then do a comprehensive oral examination. This examination will include an examination of all oral tissues for any pathology, including oral cancer. We will then measure your gum pockets to begin to assess the magnitude of your periodontal disease. We will also evaluate tooth mobility and your bite. If a complete set of dental x-rays is not available from your family dentist, then a full set will be taken at that visit too.

A follow-up consultation visit will than be scheduled to review all of the findings from the comprehensive periodontal examination, and a personalized treatment plan will be presented to you at that time. We will always answer any and all questions you have, because we firmly believe that an informed patient has the best likelihood for success in treatment. Remember, we are a team working together to achieve optimal periodontal health.
